Is it possible to have a duty of candour prompt when recording an accident or incident so that it is clear that appropriate actions have been taken if an incident or accident meets the criteria?

At the moment, nurses are having to create a wound care plan for every bruise, reddening or pimple. It doesn't make sense, care staff need to be able to document on a body map and they are the one's seeing the residents' skin.
