In Gloucestershire we have initiated Mouthcare Champions and are looking at reducing infections and resident wellbeing linked with mouthcare, including acess to dentists.

I would like to suggest the development of a generic configurable form template within PCS that can be used to create a wide range of documents, such as consent forms, leave requests, agreements, acknowledgements, and other service-specific forms....

The new layout of the care plan section where everything is pushed to the right is awful. Please change the layout back to what it was before.

For example when choosing check equipment to add a choice list, hoist, sling, wheelchair, wheelchair belt instead of going into separate icons

Every category on Plan Care Day has an "Other" option for things that don't fit into the options offered, except for Personal Safety. Can we have an "Other" button there too please.

For residents who are under Adult Social Care and a legal professional icon for residents who are under the mental health act/CTOs.

Could you add more options of specialists? Mental health team, crisis team, funeral directors, etc.
