I think there should be a 'view history' option on planned care, so that it is possible to look back at any point in time to check what the care instructions were for the resident. For example, this may be needed for investigation purposes.

An action icon added for irritable behaviour.

A choice icon added to the MCM - Such as choice of clothing, choice of drink etc.

An action icon required for when a service user is feeling unwell - as the icons on the system only relate to if a service user is sick or in pain.

Kindly include a PIN button function on shift handover for important information to be handed over for at least couple of days or weeks for all staff to be informed about it e.g. A resident's IDDSI level has change.
