Could we please add on a review section in the PBS assessment, this will be beneficial for keeping a log of evidence as justification for the PBS and ensures that the details in PBS remains relevant.

When clicking an emoji for mood I do think unhappy should provide a box like the very unhappy to explain why our resident is unhappy and what have we done to change this.

Someone who is under the weather is unwell but the only tile available would be sick or pain tile, but this does not cover generally unwell.
