SBARD

Can we have a bit mire information on the SBARD form? This is great, but there is a paper form I have seen with loads more detail and it would help staff to capture the relevant information. This information is what's on the current RESTORE2 Form.

Situation: Provide a clear and concise overview of the relevant issues. When calling 999/111 state that you are a professional and provide the resident's name and DOB. State the concerns using facts (eg. BP is low, temperature is X, pulse is X, appears to be confused/drowsy).

Background: State the relevant history. The resident has X medical condition and does/does not have a DNACPR care plan agreed. State any recent medication changes, last GP appointment. The resident's condition has changed in the last X hours. The last set of obs was at X time. Their normal condition is X. The resident takes the following medications...

Assessment: Summarise the facts and the best assessment of what happening. State if you think you have identified the problem OR if you're unsure. State if you have given any medication to treat symptoms, or taken any action (sat the person upright). State if the person appears to be deteriorating.

Recommendation: Ask what immediate action you can take or state what you need from 999/NHS 111 (ie. I need you to come and see the resident).

Decision: We have agreed that.....
