Can we add a bespoke form to capture relative reviews, from a Governance perspective we should be able to view reports as completed, as opposed to going into each individual plan/care notes to find information

We would find it helpful if there was a medical condition assessment. For example an Epilepsy assessment where it could be explained what the service users seizure looks like and how to assist recovery, as all seizures present differently.
