Add slider - taking carer to falls report

Like the behaviour slider - taking carer to a more detailed report to complete with prompts. Could this also be made available when reporting falls - this would stop paper copies / desk top only completion of incident/accidents - allowing more details to be recorded at the time of the event eg) contributing factors, how was resident, physically and in their response, what equipment/aids and where in the room etc.
