Discharge Summary

Create a discharge summary for those residents being discharged into a different care facility. Items to include, MUST, WEIGHT, SKIN INTEGRITY, DISCHARGE MEDICATIONS, ANY INFECTIONS ETC. This will make it easier to discharge a resident ensuring the next placement have correct, up to date information.
