Neuro Obs Following Unwitnessed/Head Injury Fall

We would like to bring you your attention an issue with the PCS system where we feel it does assist us in meeting the NICE protocols with regards Unwitnessed/Head Injury Falls.

Currently there are 4 options that we can choose from, however none of these options are what we need.

We have spoken to the support team who suggested we send this into you so that the opportunity can be looked at.

We carry out neuro observations when an unwitnessed fall or a fall with head injury occurs, and as per NICE guidelines, we need these obs to be set-up for 24hrs only, as follows:

- every 30min for the first 2 hours

- hourly for the next 4 hours

- 2 hourly thereafter, up to 24 hrs since the fall happened.

We are suggesting that the system could be setup to allow some parameters to be set such as:-

• Frequency of Checking as outlined above

• Length of time to do the Obs ie 24hrs

• Trigger for when Obs may be used as this is not always even if it is manual that an ob is triggered when the fall is registered.

We currently are having to use a manual system on paper to adderss this and this can only lead to error and reporting inaccuracies in the future.

I attach the form we use which relates to the Glasgow Coma Scale and flow chart which may aid whoever looks at this.

Please advise on how this may be addressed.
