Last Offices tile

We need a Last Offices tile to record care given at this important time. Currently we are having to discourage care staff from clicking a variety of personal care tiles which are inappropriate to be using for this care event. A Last Offices tile should mostly be a free text entry should also not include a Level of Care Required or Happiness rating
