Add Root Cause Analysis (RCA) option to wounds assessment

For wounds that are acquired, we are required to complete a root cause analysis. This identifies the causes that could have potentially caused the wound and highlights actions and lessons learnt. There could be an option to complete an RCA for those wounds in order to maintain compliance. See example below.

This is a mandatory task for all acquired wounds and I am sure that all managers will use this tool
