The nurse/senir should be alerted if a soft sign is recorded. Likeon the alert dashboard it should track a soft sign recorded to allow the leads/mangers to actquicker in the event the staff have not communicated a soft sign. This has also been discussed on a CCG meeting wherby they also believe the soft signs should alert.There is a focus on early intervention to reduce deterioration and hospital admissions
Yes Soft sign alert to go to Nurse/ Management to notify and trigger NEWS2 automatically to be performed and SBARD completed if needed.
