Many wounds are unfortunately categorised incorrectly when first discovered and there is currently no way of updating the 'Description' of a wound at the top of the treatment plan once populated. In addition, if there is a wound that deteriorates, for example from a blister to a pressure ulcer or a moisture lesion to a pressure ulcer, then these will not be visible in the pressure injury analysis.
A new treatment record can be made and the former archived but then the progression of the skin integrity issue is lost and becomes extremely difficult to track.
when you change the description of a wound such as a category 3 from a 2 it should only update from the point of change and not the whole record as it is difficult to known when it deteriorated.
