  That is great suggestion and very important part of evidence especially when it comes to meet the regulation and safeguarding outcomes. Also it would be helpful to have separate columns for signing off care plans and assessment tools/ risk assessments

would it be possible to be able to see that the staff team have read a care plan so that should they not follow this they cant say that they did not know !
