similar to the respite care plan within pcs, it would be handy to have new care plan form that could be tailored to people for end of life, this could overwrite their current care plan and save time as well as making it more reflective and up to date for the resident
This is a great idea. However, what I do is to create a general health decline care plan if the resident has not been confirmed as end of life and it can equally be used as an end of life care plan. This shows that staff was aware the resident's health was declining/deteriorating and care-plan was adjusted to accommodate their present needs.
