Unusual Events should have a section for follow up / managers sign off

I would like for unusual events to have their own review form - like an accident/incident review - As these events do often require some investigation and follow up.

It would be factually inaccurate to log them as incidents/accidents , therefore we have nowhere to add any additional information or follow up from an unusual event

You could have an unusual event which requires investigation but there is nowhere for them to evidence the investigation, as these are currently left open with no option for follow up.
