SSKIN BUNDLE enhancement

We currently follow the SSKIN bundle framework in practice, using both the SSKIN chart and a body map to document skin checks, pressure-relief interventions, and any areas of concern. At present, our workflow requires staff to complete the SSKIN chart and wound body map on paper, and then cross-reference this information with the resident’s Wound Care Plan in PCS.

While this process is safe, it creates duplication and increases the risk of inconsistent documentation because the paper SSKIN bundle and the digital wound plan are not directly linked.

To improve accuracy, efficiency, and audit quality, it would be beneficial if PCS included a dedicated “Medical / Clinical Care” tab where we could:

  • Build a digital SSKIN Bundle Plan for each resident

  • Automatically link wound entries (existing or new) to the relevant SSKIN actions

  • Upload or draw on a digital body map

  • Track trends in skin integrity, pressure area risk, and healing progress

  • Generate an audit trail showing how SSKIN interventions are supporting and improving wound outcomes

  • Set up alerts/reminders for repositioning, hydration prompts, moisture checks, and barrier cream application

  • Standardise documentation for CQC and tissue viability audits

This integration would streamline the current paper-based process, reduce errors, and allow nurses and senior carers to complete holistic skin assessments within one system. It would also strengthen clinical governance by providing clearer, more accessible evidence of how the SSKIN bundle is being implemented in line with each resident’s wound plan.
