Add a customisable icon for planned care

It would be useful to add a custom icon on planned care where we can place a photo and the title, like creating from scratch but in a simple way, as sometimes we can not find the most appropriate action, for example I need to add on planned care for a person to check this equipment: aircast cryo cuff cooler, is used for pain relief, mainly in rehabilitation. I believe will help with some specific creams, or eye drops or medications, or machines that are used in our days.
