Tailored wound charts for residential homes

It would be a very good idea to have tailored wound charts for nursing home and residential care homes. As a residential care home we are not to complete the tissue state section as we are not trained to complete this correctly and appropriately. It would mean our wound charts are completed in full without missing information. The district nurses do not give us this information and do not have time to sit down and go through the charts themselves to complete this section.
