More mental health related subjects within initial assessment

I would like to see more Mental health care home appropriate headings and subjects within the assessments and initial assessment particularly.

There is no assessment for risk to self / others.

There is also no assessment for vulnerabilities relating to mental health, e.g. risk of exploitation too.
