combine assessment and care plans

Frequently, we encounter significant challenges stemming from inadequate triangulation of information incorporated into care plans and assessments, which often do not align, or one is reviewed without consideration of the other.

For instance, if the assessment—such as a falls assessment—was completed, and the care plan / fall prevention plan was included on the same form this would save time and reduce the errors and other example is the mobility assessment also containing the care plan at the bottom, this would effectively reduce the number of reviews required, ensuring that information is updated concurrently.

so in essence each assessment would link to the the care plan within the same form or become one document - making them concise and reduce the need for both documents
