Add "Location" to Care Notes/Actions

Is it possible to add a location or community field to care actions and notes?

This would be a huge help for us. Our residents often move between communities, and it's tough to track down specific information. Having a dropdown menu to select the community where a note was made would make searching so much faster and more accurate.
