Incident reports

ideally we would like the ability to show the processes we go through with the incident reports to ensure all aspects are considered.

Witnessed/unwitnessed. Risk level (high,medium, low) Type of incident: Minor/Major/Head injury.

Medications - on more than 4? Blood thinners?

Obs completed.

Contributing factors considered? Environmental/medication/resident/Transfer error/mobility issue.

Time of last meal or snack?

Change to care plan required?

Safeguarding required/family informed/CQC/Riddor - Date reported?

This will help point those completing the incident that they have completed all actions.
