attach letter to care plan eg Falls team to mobility care plan

Instead of just having professional letter in the doc section, maybe have a section in the care plan so it can be attached to the relevant care plan, the letter scan section needs to be broken down to more sections.

    I agree. It would be very useful to be able to link scanned docs to the relevant care plan such as dols authority to mental capacity plan.
