On the current incident report it only mentions immediate actions and then has a follow up box. It would be good to have a section that states 'lessons learnt' and also the option to add more detailed follow up actions. The incident report does not date/time/staff stamp who has updated the incident report, so does not evidence at what points it has been reviewed (only when logged and signed off). This would be good as not all incidents can be signed off within 72 hours especially if there are any investigations are required. some incidents also have more than one person adding to it, so would be good to identify at what points who has added what.
