Removal of prepopulate care note on all tiles

Removing the prepopulated care note such as on clothing tile it says 'dressed' - instead when clicking into 'change' automatically the system brings up the care note box so that the carers can record a more person centered note like there would be if it was hand written. Carers wouldn't have the option to rely on the prepopulated note overall better detailed outcomes
