"Keeping safe in bed (Bed rails)" form has some issues.

I've been asked by our care quality lead to say that the "Keeping safe in bed (Bed rails)" form could benefit from some improvements as the method that they currently use for assessment of the need bed rails is far more rigorous and detailed. The form they use is attached. They are especially concerned that the PCS form allows a decision to be made as to whether bed rails are required or not by the person filling in the form.
