Simple hospital passport

Ambulance service has moaned saying that the hospital pack is far too comprehensive in an emergency situation. The hospital passport should cover information such as risks to be aware of, DNACPR, medication, diagnosis and allergies not likes and dislikes, this is fine for an admission to give to the ward but it'd be helpful to have a system generated hospital passport to prevent having to manually update information that is already being updated on the system.
