Scoring of choking & Dysphagia risk assessment

The current scoring system of the choking and dysphagia risk assessment is scored on 'danger signs' As a nurse, this does not make any sense. I feel it needs to be low, medium or high risk so that the nurses who are using the assessment understand the outcome of it.
