on paper we have do body maps weekly for our residents i have spoke to a member of the team who have confirmed unless this is a wound or infection this is not a option on pcs maybe there could be a body map that carers can click on and complete to track skin weekly
I agree with you .The skin inspection ,body map should be separate from wounds .
We have residents with ongoing bruising due with skin condition and anticoagulants ,we should review regularly for monitoring ,when we are doing monthly audits ,appears as we have lots of bruises ,but they are only bruises .
