Purpose T - there is no such thing as an ungradeable wound

Recent tissue viability training and up - to - date best practices and guidelines state that there is no such thing as an "unstageable" or "ungradeable" wound - however on the PCS' Purpose T, it is still listed as an option under the pressure ulcer categories. The staging stops at stage 4, this should perhaps be updated in line with NICE guidelines

    Good morning,

    Upon reviewing the NICE guidelines on their website, it appears that unstageable is still listed as an option relating to wounds as of January 2024. Would you be able to provide any links or documents which highlights this may have changed?
