Most care settings will require either 6 month or yearly reviews of care with the resident/service user alongside a family member/LPOA/Significant person
I really think this is important to document in PCS that this has taken place and any actions/outcomes identified. I haven't yet found the best way to record/evidence this and wondered if this could be in Outcomes - i note you can link to a risk assessment but linking to a care plan would also be beneficial as you could name this 'annual review'
I agree this is used in other providers and would be easily used and make tracking of dates so much easier
Great idea, that should be form available under assessment, form should highlight when review of care is due
