Have short document that can be completed then linked to any care plan etc (same formula that a risk assessment is linked to a care plan) whereby it can be easily evidenced the level of involvement a resident or family have had in the creation of that information. Easy little tick box, detail, save, attach.
example -
Resident - completed ( ) involved ( ) read (x) agreed (x)
Family - competed ( ) involved (x) read ( ) agreed (x)
Comments - "Sister, Tina, phoned the home on 15/11/24 and provided information included in this plan"
