On the risk assessements would be handy to indicate if the person has capacity to undersatnd and then be able to link this to a BIA.
secondly - name of the representative as there may be more than one individual for seperate areas and would be good to record who and then link this to a system handover note that the person made a decision in support of an individual in our care.
I know this was a question asked in a cqc inspection and would then make this very clear who said what and who was involved when and that discussions took place.
