written completion of the incident forms

I feel the incident forms need to be changed. I reviewed the incident forms that staff filled in, however they are unable to give a detailed explanation of the incident, they can only select icons. On reviewing I am completing the detail on the incident form but I am unlikely to have witnessed the incident. I feel the staff need the form that i can access so they can type a detailed account and what they have done about it, but the information relating to who has been informed such as commissioners, relatives, CQC etc needs to be on the reviewing section of the form.
I find myself repeating the information that staff have recorded, unless i have witnessed the incident.

This is very time consuming when you support adults with complex needs.
