Missed Action Rationale

In our nursing home, we do a daily PCS audit and we needs to rationalize the missed actions. If there is an option on the missed action to put the rationale for the missed action, it will be very helpful. As CQC would like this as well. And anyone who wants to see the reason for the missed action, can see it there on the system itself.
