intervention recording

Hi,
with the incident reporting, where physical intervention or holding is identified , would it be possible for a box to open, so that we can document which staff were involved , what level of intervention or hold was used and what staff were holding which limb.
can we also have a box to indicate if staff were offered a debrief after physically intervening or holding residents.
