Addition of end of life specific care plan. Following GSF guidance this care plan should offer various sections to cover the different needs during the end of life stages. This care plan would sit on the system to be implemented at the appropriate time but could be completed and amended throughout the residents life. At present there is not a detailed section in which this can be recorded.
