Delirium assessment

Delirium needs to be treated as soon as possible, but can be missed, which can lead to people becoming very unwell. It would be useful to have a specific tool to use if a person displays unexpected behaviour challenging to others, or there are indications of unwellness which need further investigation. It could suggest action to take, which would support non-clinical staff when speaking to clinical staff and seeking support. It could also suggest need to carry out other PCS assessments. Delirium is known to be a high risk for older people, with a range of medical conditions and related medications used to treat conditions. Having this in PCS would raise awareness and support staff to act promptly.
