Not being able to see the outcomes in some of our tenants own files. This is making Quality audits difficult to manage. Plus staff are having a hard time locating them to review and being told in QAs that they haven't been completed when they have.
We are also experiencing issues with outcomes, raised a support ticket 2 months ago and now an investigation has been opened. It appears this is an area that PCS should improve on since they offer a service that does not work properly
