this would be useful as it would allow correspondence with professionals such as SALT or physio to be attached to the relevant care plan and allow for quick reference.

it would be great if you could attach a document straight to the relevant care plan or risk assessment so all is together instead of in the separate section on scanned details, this would look more professional if all was together for each section
