Sensory Screening and assessment & urgent review of Oral assessment to align with guidance - Oral health matters NHS

At present time you dont have

Vision screening Form, Hearing Screening Form nor a PAL assessment of what cognitively a person can do. These are integral elements to personalised care and without such impact communication in all areas for a person. I would like to see these on this system, we have uploaded ours and do our own screen at present as this is not intergrated into PCS as yet but i feel they should be.

I also feel the Oral assessment should also include a screening process in the pre assessment stages and then routinely every 6 to 12 months should be completed to make sure people have identified needs and the tools available to them. We have taken our screening assessment from NHS oral health matters, hearing from RNIB which you can view online and PAL if you have the jakie poole resource.

Would be good to see this integrated in to this system and to link up with oral and mouth care, glasses and sight and hearing aids link to hearing loss all of which impact communication.
