Last days of life care plan

We are in the process of applying for Gold Standard Framework accreditation for the first time. MCM has the needs based coding (blue, green, amber and red) and has the Edmonton frail scale that can be used to identify the code. Gold Standard Framework are asking for a personalised care plan for the last few days of life. We do not have that. Is there any way that can be added
