Care Plan - Domiciliary Care

A more domiciliary care friendly care plan, the current care plan does not suit the domiciliary care setting in my opinion as a lot of it's content isn't relevant domiciliary care, too many sub headings makes it difficult to locate certain information quickly. Carers should be able to pick up a care plan and be able to go straight to the information that they need.
