Improvement of fluid input and output assessment and management to prevent dehydration in vulnerable adults

Fluid intake and loss - fluid charts detail how much is taken in however what they dont show is input vs out put so for iinstance they may take 2000ml fluid - but when they have sickness or diarrhoa the loss then is more significant because its likely there is at leats a 1000ml lost through illness - then if this is 3 or 4 times a day - fluid intake then is in the minus and they are loosing quicker than they are gaining. We did used to use this to increase intake in hot weather and severe illness but your charts are not reflective of this and you also do not have a gulp dehydration tool which could link or at least tie into this to show what is coming in, what is going out and the risk level - which over a year will change monthly as sometimes in winter months a person could be low level - but in illness and severe hot weather this then bumps into HIGH risk. Please can you review this to reflect this as an urgency - to tie into legislation and guidance in supportinig older peoples fluid hydration care - i have tools i am happy to share with you.
