On the risk assessments would good to add why there is a risk or a box - so if they are at risk for example of dehydration but have dementia and diverticular disease and sensory loss - this would then support the impact and likelihood as there is a history.
This also then shows that we have identified what it is, why its important, and ion aswell as emergency action that staff or seniors might need to do to reduce and manage or remove in terms of risk itself, for which there is no box for this either. This would then ensure there is a clear route of what to do and when and have clear guidance that staff and seniors can follow.
I do think that all staff should be able to access the risk assessments but not edit these. Is this something you can also do - in a way they can access from the handset itself and that this risk flags on the main screen?
