On the Nutrition assessment it would be helpful to have a question around whether the person has mental capacity/can understand any texture modifications/is happy to comply with the recommendations/can decide not to follow these despite being advised of choking/aspiration risk/if someone else has to make a decision due to lack of capacity or understanding then how is this to be done/best interest/LPA.
Unfortunately incidents happen when the individual or their family decide that they will not follow the guidance or that "it will be ok just once" - but people can die in these instances.
