Tracking changes in care plan writing.

When a nurse starts writing a care plan and has to lave half way through she/he will save it. Then if another nurse comes along adds more to the care plan and saves, it saves as the original nurse. As a nurse with a PIN number i am very protective of it. If the 2nd nurse was to come along, change everything I've put on the care plan, then something happens to that resident, I would be the one to blame as I had "written" the care plan. Is there a way that ALL changes can be tracked so that any changes can be attributed to the 2nd nurse?? I know I could review the care plan and that would show any changes.
