Links between Personal Behaviour support plans and current assessment

The links between the Personal Behaviour Plans and the current assessment do not work for us. For our residents who have capacity their current assessment and care plan are written in the first person. A resident who has issues around her behaviour with others would not write her own behaviour plan therefore it is written in the 3rd person which changes the linked current assessment. In addition the links are not useful - possible triggers on the plan is linked to depression and anxiety. The Behaviour Plan is an extension of the information in the current assessment - the current assessment tells us what we are managing and the plan tells us how
