planned care signing off

When Planned care is in place for example at 12pm there is fluid, pad, check ok and repositioning all on the one time, say we just give a resident a drink, why does it say we have done the pad, check ok and repositioning as well they all sign off at the same time. Can this be looked into as it is taking a lot of time and extra notes for our care staff when planned care is supposed to help
